View Birmingham's Charitable Food Service Ordinance    General Instructions -- Please Read Carefully        Your Access Code will function as a PIN and will be used by you to make or cancel reservations. Please protect it.     Along with your application for an Access Code, you must provide two documents to the City of Birmingham Traffic Engineering Department. We will require:    (1) A color photostatic copy of both the primary and secondary applicants' driver's licenses    (2) A photostatic copy of your organization's current Health Department Permit       Once you have submitted your request for an Access Code, please provide us with these two documents. These documents may be attached and sent by email or may be brought into our office in person. If emailed, please make certain you include your organization's name (not your Access Code) so that we know to whom it belongs.
